We've worked out at least a possible way this can occur from using uninitialized AnimatedNormals values when changing LODs. Ensuring all AnimatedNormals are initialized appears to fix the problem.
We do not have a solid repro internally, but we have a licensee that was able to get a repro, and we have a speculative fix.
The problem seems to occur when spawning at one LOD and then rapidly transitioning multiple LODs, e.g., from a camera change.
It appears that it only happens if you have ISPC enabled (default on most platforms).
